PHILADELPHIA – September 14, 2017 – Turn-Key Health (TKH), an advanced illness population health management (PHM) company serving payers and their members, announces the release of the first part of its three-part white paper, “Effectively Addressing the Impact of Advanced Illnesses.” This series expands the understanding and analysis of the 12-18 month time period during the last stages of serious illness, and provides an introduction to new, innovative models of care that successfully address these challenges for 1) health payers, 2) providers and the U.S. healthcare system, and 3) consumers.
Part One, “Recalibrating Population Health Management to Include Community-Based Palliative Care,” examines specific issues for health payers, and how Turn-Key’s innovative, proprietary model Palliative Illness Management™ (PIM™) augments traditional PHM to deliver improved quality of life for members and caregivers facing the many burdens associated with advanced illness.
