Turn-Key Health Chief Clinical Officer Terri Maxwell Joins Strategic Solutions Network Panel, Discusses Best Practices and Alternative Care Models for High-Risk Patients

PHILADELPHIA – March 13, 2017 – Turn-Key Health, a population health management (PHM) company serving payers and their members with advanced illnesses, announces Terri Maxwell, Ph.D., APRN, chief clinical officer, Turn-Key Health, will participate in a panel discussion, “Innovations in Models of Care and Care Transitions,” 4:30 p.m., Tuesday, March 14, 2017 at Strategic Solutions Network (SSN) Best Practices for High Risk Populations, Care Transitions & Palliative Innovations, March 14-15, 2017, Omni Orlando Resort, Championsgate, Orlando, Florida. Panelists will focus on alternative care models for improving the care and quality of life for patients with advanced illness.

“This panel explores opportunities to better address the needs of high-risk, high utilizers of healthcare, the costliest population cohort that is usually battling multiple chronic illnesses,” says Dr. Maxwell.  “Our Palliative Illness Management™ — PIM™ model focuses on the importance of predictive analytics within PHM programs to identify these patients early on in the disease trajectory.  It then extends the value of PHM by introducing community-based palliative care teams to engage these members and caregivers in informed decision-making that avoids non-beneficial treatment.”
