“This is an opportunity to lead the exploration of the most innovative, team-based care delivery models, with discussions that focus on programs to enhance patient care and foster effective, timely care transitions,” says Myers. Turn-Key Health focuses on patients with complex care needs and life-limiting illness, and creates a network model to align community-based palliative care providers, payers and members through technology and process.

Myers continues, “In today’s challenging healthcare environment, systems and payers struggle to meaningfully engage with patients and members throughout their service areas. Our model provides an efficient solution to address this issue by leveraging community-based network providers, which delivers appropriate engagement, reach, and frequency to the most vulnerable patient / member populations.”

The panel, “Improving Care Transitions Between Settings to Reduce Avoidable Readmissions and Improve Outcomes,” will be moderated by Henry W. Osowski, managing director, Strategic Health Group, and also includes panelists Jennifer Strohecker, PharmD, BCPS, director, Medicare Pharmacy Clinical Operations, Molina Healthcare and Gregg Pane, medical director, National Medical Policy, Aetna.

Myers adds, “I look forward to the exchange of innovative care solutions to solve key issues, especially as health systems begin to shift towards value-based payment models. Integrating non-medical palliative services bolsters medical services to develop positive health outcomes and patient experiences for those with life limiting and advanced illnesses.”
