Palliative Illness Management™ - PIM™

A Unique Model that Improves Member and Caregiver Quality of Life

Palliative Illness Management™ is Turn-Key Health’s rapidly scalable solution within insured member populations. This innovative, high-touch, home-based program engages members to better align their goals of care with an individual approach to healthcare services. It is a solution that reduces unplanned care and non-beneficial treatment.

The PIM™ solution is delivered by utilizing predictive analytics/AI for appropriate member identification, by training and deploying networks of palliative care professionals to engage with members in the community, and by managing palliative member engagement in the PIM™ Platform.

Through its proprietary platform, PIM™ brings a systemized process of palliative care delivery, providing consistency across geographic regions. This specialized solution seamlessly integrates with existing case and utilization management programs.

Palliative Activation Scale

PIM™ utilizes a Palliative Activation Scale™ (PAS) to measure a member’s propensity to adopt a palliative care approach. PAS™ evaluates three key domains: care alignment, social determinants of health, and illness trajectory. The interaction and positive progression within these three domains represent leading indicators of positive quality outcomes. Turn-Key utilizes the elements in this scale to guide appropriate interventions and interactions to affect positive outcomes for patients.

Provider Engagement

Turn-Key Health's palliative care teams operate within existing healthcare extending the reach of physician practices and the medical home. PIM™ provides practical support that complements and enhances care management.

Palliative Care Teams

Turn-Key has a national, proprietary network of specialized palliative care clinicians - comprised largely of nurses and social workers - that reside as a separate service within local hospice and palliative care organizations.

Focusing on palliative care outside the acute care setting, they address disease understanding, provide symptom control/treatment options, complete goals of care conversations, and provide advance care planning documentation. By aligning treatment with individual preferences, these clinicians improve member and caregiver satisfaction.

Members

PIM™ focuses on improving member and caregiver satisfaction by enhancing care coordination and quality, improving knowledge and understanding, and establishing goals of care for shared decision-making.

This results in quality of life improvement, a reduction in non-beneficial treatment and unplanned care, and a decreased expense. Members benefit from diminished symptom burdens and condition exacerbation through improved medication adherence.
